Data will be used only for the provision agreed upon services and will not be shared with any other parties without consent.  Clients have the right to see the data I have and request any data can be amended or deleted.  The information I will collect is contact details and background information from school and parents.  Assessment reports will be stored electronically on a computer which is password protected.  Client’s assessment record forms will be destroyed once the final report is shared with parents.  Reports will not be shared any an third party unless I have parental consent.
